Administrative Resources for Volunteers
As an IOPO Volunteer, it is important that we keep track of the number of people we reach in our educational and community outreach events.
In addition, as a volunteer, you're expenses when performing IOPO requested programs can be reimbursed, so as part of completing the form below, be sure to report your mileage for reimbursement at the current IRS approved mileage rate for volunteer activity.

This form must be completed by the 5th of the Month following your participation in any IOPO activity.
